2010 Camp Forms and Packing Lists

These forms must be filled out and returned to the Camp Roger office along with the final payment at least three weeks before your child arrives at camp. You should also recieve copies of these forms with your registration confirmation.
Parent Packet is the parent's survival guide to YMCA Camp Roger. the planning suggestions, and camp policies are among the helpful information in this document.
Health History Form this is required for every camper. The information provided must be less than two years old.
Waiver and Release another must for every camper. To ensure they can participate in the fun-filled activities throughout the week!
Camper Information Sheet there are two forms, one for the parent and another for the camper to fill out. It provides the camp director and counselors an opportunity to know more about the campers prior to their arrival.
Camp Admission Card must be brought with you to check-in. This form provides the names of individuals who may pick-up your camper at check-out as well as the Camper Code of Conduct agreement.
